Wellness Program

The goal of the Three Oaks Equine Veterinary Services Wellness Program is to provide each horse with a solid foundation of care.  By working closely with our veterinarians, you can build on this foundation to tailor a program that is appropriate for each individual horse’s health and wellness needs.

*The acupuncture scan consists of the evaluation of individual acupuncture points associated with potential musculoskeletal problems.  To evaluate soundness, the horse will be jogged in hand in a straight line and circling both directions.

** The level-one motorized dental includes the removal of sharp enamel points, placement of a bit seat, and the evaluation of incisor and molar balance.  Sedation is not included.

***The Annual West Nile Virus Vaccine, Rabies Vaccine, and Coggins test may be performed at either the Yin or Yang visit depending on your horse’s current preventive health care schedule.

 

2017 Wellness Program

-Terms and Conditions-

  1. The Wellness Program may be purchased between January 1, 2017 and May 15, 2017. All 2017 Program products and services must be used prior to December 31, 2017.  If the products and services purchased in your selected level are not utilized in the enrollment term, there are no refunds, substitutions, transfers, or credits.
  2. Horses enrolled in this program must be in Three Oaks Equine Veterinary Services, PLLC practice area and be one year of age or older. This program applies only to the enrolled horse and is non-transferable.
  3. Clients wishing to enroll in this program must be in good standing at the time of enrollment. Full payment must be received at or prior to the time of service.  For your convenience, Three Oaks Equine Veterinary Services, PLLC accepts cash, check, Visa, MasterCard, American Express, and Discover cards.
  4. If the enrolled horse is sold, the Wellness Program benefits will be honored for the new owner provided the horse remains in the Three Oaks Equine Veterinary Services, PLLC practice area.
  5. If a horse dies during the period in which they are enrolled in the Program, any unused balance will be donated in the name of the horse to the American Association of Equine Practitioners “Foundation Equine Memorial Program”.
  6. The Wellness Program is exclusive of any other promotions or discounts offered by Three Oaks Equine Veterinary Services, PLLC .
  7. During the enrollment period, the level selected will not be subject to change. Program levels may be changed at the renewal of the Wellness Program the following year.
  8. Farm call fees, sedation for dentistry, booster vaccines, and any additional services not specified are not included in this program.

-Fecal Reminder-

Please consult with your veterinarian regarding the appropriate time for your horse’s Fecal Parasite & Sand analysis.  Collect 2-3 fresh fecal balls that are minimally contaminated with bedding and place them into a Ziploc bag labeled with your name, your horse’s name and the date of collection.  The sample should remain refrigerated or chilled until it is delivered to Three Oaks Equine Veterinary Services.
